Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.

A monitoring inspection was initiated on 10/08/2021 and concluded on 10/18/2021. The director was contacted by email to initiate the inspection. There were 39 children present and 2 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-185-130-A
Based on record review and interview it was determined the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence:
1. There was no immunization record available for child #3 at the time of the inspection. Child #3 was present during the inspection.
2. Staff confirmed that these records were not available
Plan of Correction: Staff will ensure each child's record contains immunizations records.
Standard 22VAC40-185-140-A
Based on record review and interview it was determined the center did not ensure each child had a physical examination by or under the direction of a physician within the required time frame.
Evidence:
1. The records for child #2 and child #3 did not contain documentation of a physical exams.
2. Staff confirmed that there were no physical exam documents for child #2 and child #3 available during this inspection.
Plan of Correction: Staff will ensure each child enrolled has a physical exam within the required time frame.

Based on record review and interview it was determined the center did not ensure that there was a record for each child enrolled at the center.
Evidence:
1. There were no records available for child #2 and child #3 at the time of the inspection. Both children were present during the inspection.
2. Staff #1 confirmed that these records were not available.
