Inspector Notes

An unannounced, on-site monitoring inspection was completed on 2/19/2025, as a part of the licensure period. The on-site inspection began at 3:35 pm and ended at 4:20 pm. The inspector reviewed compliance in the areas listed above. There were 25 children present and 2 staff. The inspector reviewed 5 children?s records and 2 staff records. This inspection included a document review, tour of facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-340-C
During the stated hours of operation, there always shall be on the premises when one or more children are present one staff member who meets the qualifications of a program leader or program director and an immediately available staff member.

At time of inspector's arrival there were 25 children in care alone with staff #1. Staff #1 stated that staff #2 was in the restroom. Staff # 2 had not returned at the conclusion of this inspection, which was 45 minutes in duration.
Plan of Correction: Talked with both counselors and expressed that they need to communicate with their Youth Director, when not feeling well and/or find them self alone with children in their care for an extended period of time.
Standard 8VAC20-780-350-C
When children are in ongoing mixed age groups, the staff-to-children ratio and group size applicable to the youngest child in the group shall apply to the entire group.

Staff #2 was observed supervising 25 children (youngest child 8 years old) from approximately 3:35 - 4:20 pm.
Plan of Correction: There is a priority of getting a councilor hired and to be placed at this location. Having conversations with my region hiring Youth Director.
