During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site monitoring inspection completed on 03/19/2026, as a part of the licensure period. The on-site inspection began at 2:40 pm and ended at 4:10 pm. The inspector reviewed compliance in the areas listed above. There were 69 children present and 4 staff. The inspector reviewed 5 children?s records and 6 staff records. This inspection included document review, tour of the facility, interviews, observations, and measurements. A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards. Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Staff #5, did not have documentation of requesting a criminal history check or a central registry check from the state they had lived in the preceding five years. Staff #6, present during this inspection, did not have documentation of requesting a central registry check from the state they had lived in the preceding five years.
During the inspection, there were no staff consistently in the corridor where the children were using the restrooms. These children were intermittently unsupervised for approximately 5-8 minutes.
The injury report dated 3.3.26 for child #8 only contained one of the two required signatures.
The injury report dated 3.9.26 did not contain the time, date, or method
of parental notification.
Child #6 and #7 were observed walking around the cafeteria eating and drinking during snack time.
Staff #2, #3, and #6 did not have documentation of certification in CPR.
Current certification in first aid appropriate to the ages of children in care.
