Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A subsidy health and safety inspection was initiated on 01/14/2021 and concluded on 01/26/2021. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview, the vendor failed to ensure that all staff who work directly with children shall have within 90 days of the date of employment current certification in CPR.

Evidence:
1. Staff 1, hire date 06/29/2021, lacks CPR certification.
2. Administrative staff confirmed that staff 1 did not have CPR certification.
Plan of Correction: Administrative staff stated that a CPR class is being set up for staff 1 to attend.
