During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced , on-site monitoring inspection was initiated on 01/29/2025 and completed on 01/29/2025. The on-site inspection began at 3:25 pm and ended at 4:35 pm. The inspector reviewed compliance in the areas listed above. There were 42 children present and 4 staff. The inspector reviewed 5 children's records on-site and 3 staff records electronically on 01/30/2025.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 02/07/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-245-J-3
Any child for whom emergency medications (such as albuterol, glucagon, and epinephrine auto injector) have been prescribed shall always be in the care of a staff member or independent contractor who has current MAT certification.

There was no one on site with current MAT training until 3:45 pm. There was a child in care (child 2) who has emergency medications prescribed.
Plan of Correction: A MAT training counselor needs to be at site at all times. A counselor at CRE will be signed up to take this course to help maintain this correction.
Standard 8VAC20-780-60-A-8
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following information:
*A written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

There was no written allergy care plan on file for child 1 who has a diagnosed food allergy. Child 1 was in care during the inspection.
Plan of Correction: During parent pick up on 1/29/25, staff talked to parent that it is imperative to haven an Emergency Action Plan at site for this student.
Received the document the following day. This has been added to student's file and medical binder.
