Inspector Notes
A complaint was received and initiated on March 18, 2024 regarding lack of following ratios, sanitation of tables and evacuation drill practice. The onsite investigation was completed on May 1, 2024. The investigation concluded on May 22, 2024. During the investigation program observations were made, staff were interviewed and documentation was reviewed. The information gathered during the investigation supports part the allegations, therefore, the complaint is valid. A single violation regarding ratios was found, appears on the violation notice and was discussed with administrators at the conclusion of the investigation.

Based on documentation review and interview the center failed to ensure that required ratios are maintained.
Evidence:
1 - On March 11, 2024 between approximately 2 and 3 pm there were 68 children present, ages five to eight years old, with three staff where four staff are needed.
2 - Administrator #1 confirmed that there was only three staff present on March 11, 2024 when four where needed.
