Inspector Notes
A renewal inspection was initiated on September 11, 2023 and concluded on October 4, 2023. Staff records were reviewed at the central location on September 11th and the onsite inspection was completed on October 4th. Staff and children's records were reviewed. Children were observed in teacher and self-directed activities including meals, transitions and rest room routines. Emergency evacuation drills, emergency equipment and medication were reviewed. Violations regarding records, group size, allergy information and evacuation drills, were observed and discussed with persons in charge at the conclusion of the inspection.

2 - The written information did not include the allergy plans for any of the individuals listed.
3 - Staff #1 and #2 confirmed that the written list was not dated nor were the allergy plans available with the written information.

1 - There is not any documentation that an emergency evacuation drill was conducted in September 2023.
2 - Staff #1 confirmed that there was not documentation of an emergency evacuation drill documented for the month of September 2023.
Evidence:
1 - There is not any documentation of name, address, verification of age requirement, job title, and date of employment or emergency contact for Staff #3.
2 - Staff #1 confirmed that there was any documentation for Staff #3 in the staff information binder.
3 - Staff #3 stated that he believed another staff member had his emergency contact information.
