During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 12/16/2024, as a part of the licensure period, The on-site inspection began at 3:40pm and ended at 4:45pm. The inspector reviewed compliance in the areas listed above. There were 52 children present and 5 staff. The inspector reviewed 5 children?s records and 9 staff emergency contact information were reviewed on-site. Staff records are maintained in a central location and will be reviewed at a later date. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 1/10/2024. A POC submitted after this date will not appear on the public website.

The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises. The required postings including the license, emergency evacuation plans and routes, emergency numbers and daily schedule were not posted during the inspection.

Standard 8VAC20-780-350-C
When children are in ongoing mixed age groups, the staff-to-children ratio and group size applicable to the youngest child in the group shall apply to the entire group. From approximately 2:17pm until 3:50pm there were between 53 and 52 children present ages 4 ? 11 years old with five staff. The number children present required an additional staff.

The following nonmedical emergency supplies shall be required, one working, battery-operated radio in each building. The staff were not able to operate the battery-operated radio available at the center, during the inspection.
