During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 3/18/2025 and completed on 3/26/2025, as a part of the licensure period. The on-site inspection began at 3pm and ended at 5pm. The inspector reviewed compliance in the areas listed above. There were 54 children present and 4 staff. The inspector reviewed 6 children?s records and 6 staff records on-site and electronically on 3/26/2025. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/1/2025. A POC submitted after this date will not appear on the public website.

When staff are supervising children, they shall always ensure their care, protection, and guidance. On 2/14/2025 a six-year-old child with a known peanut allergy was unknowingly given a peanut butter candy by an adult who was not a staff member of the licensed program. The child had a reaction that required medication onsite and outside medical treatment.

When children are in ongoing missed age groups, the staff-to-child ration and group size applicable to the youngest child in the group shall apply to the entire group. There were 54 children ages 4 through 9 in care in one large group with four staff.
