Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on Tuesday, March 30, 2021 and concluded on Wednesday, March 31, 2021. The in-charge person was contacted by telephone to initiate the inspection. There were 43 children present and 9 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Based on review of children's record, the center did not ensure to obtain all information for each child's record.

Evidence: In Child#1's record (start date 9/24/2020), the two designated persons to call in an emergency if parents cannot be reached addresses were incomplete.
Plan of Correction: Center will ensure all information for each child's record is included, including emergency contacts addresses.

Based on review of staff records, the center did not ensure to obtain all information required for each staff record.

Evidence: In Staff#2's record (date of hire: 7/20/2020), the signature of person making the call for the reference check was not documented.
Plan of Correction: Center will ensure all information for hiring is complete including signature of staff member conducting interviews/ reference checks.
