This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).

A monitoring inspection was initiated on Wednesday, September 8, 2021 and concluded on Friday, September 10, 2021.The head of school was contacted by telephone and a remote inspection was conducted. There were 46 children present with 10 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of child's record and interview, the center did not ensure to obtain documentation of an immunization before child can attend center.

Evidence: In Child#2's record (start date: 08/31/21), the documentation for immunization was not found. Interview with staff confirmed this information was not available for review during this inspection.
Plan of Correction: The center will ensure to review and include all required information in children's record.
