Inspector Notes

A monitoring inspection was initiated and concluded on 9/1/2023.There were 17 children present with 6 staff supervising and 1 administrator. The children were observed during classroom time engaged in a lesson and observed during outside play time. A total of 5 child records and 3 staff records were reviewed. The inspection started at 10:25 am and concluded at 11:41 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and staff interview the licensee did not ensure documentation of the Tuberculosis screening shall be submitted at the time of employment and prior to coming into contact with children.
Evidence: The director confirmed Staff #3 (start date 8/22/2022) had no documentation of a tuberculosis screening prior to coming into contact with children.
Plan of Correction: Health department was contacted they will forward proof of negative results.
