Inspector Notes

A monitoring inspection was initiated and concluded on 1/3/2024.There were 27 children present with 4 staff supervising and 2 administrators. The children were observed during classroom time engaged in various centers as well as lunch time and nap time. A total of 5 child records and 5 staff records were reviewed. The inspection started at 12 pm and concluded at 3:45 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and staff interview the center did not ensure that staff shall obtain and submit the results of a follow-up Tuberculosis (TB) screening at least every two years.

Evidence: The director confirmed that the TB screening for Staff #1, dated 9/20/2021, was more than two years old.
Plan of Correction: The staff member has a TB screening scheduled for 1/12/2024.
