Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. Staff #5 TB was dated eight days after the date of employment.
Plan of Correction: New employees will have TB within the required 30 day window.
Standard 8VAC20-780-240-A
The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment. Staff #1 training was dated six months after date of employment.
Plan of Correction: New employees will take training within 90 days of hire.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The records for Child #1, #2, #3, #4, and #5 were missing the addresses for emergency contacts. The record for Child #1 was missing the work phone number for one parent and documentation of immunizations. The record for Child #2 was missing the place of employment for one parent and the work phone number for both parents. The record for Child #3 was missing the work phone number for both parents. The record for Child #4 was missing the work phone number for one parent. The record for Child #5 was missing the work phone number for one parent.

Standard 8VAC20-780-60-A-8
A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Child #2, who has been enrolled for three months, has a diagnosed food allergy and did not have a written care plan in their file.
