Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and a virtual tour of the program.
A monitoring inspection was initiated on 12/16/2021 and concluded on 12/16/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 63 children present, with 11 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on staff record review, an out-of-state background check result was not obtained for a staff member that has resided outside the state of Virginia within the last 5 years.

Evidence:
Staff #1 (DOH: 05/11/2021) has resided outside the state of Virginia within the past 5 years, their records did not contain documentation that an out-of-state abuse and neglect search was completed within 45 days of employment.
Plan of Correction: Child Abuse History Record Request for Child Care Personnel Employment Form was completed by the employee and submitted by the Center Director. We will put a system in place to audit new employee's paperwork and ensure all requirements are met.
