Inspector Notes
A Subsidy Health and Safety inspection was conducted on 4/25/2022 to review supplemental health and safety requirements. An unannounced self-reported investigation and monitoring inspection were also conducted on 4/25/2022 and a follow-up inspection visit was conducted on 4/29/2022, in response to a self-reported incident that was received by the Fairfax licensing office on 4/22/2022, regarding behavioral guidance and forbidden actions. Based on interviews and records reviewed the evidence gathered during this investigation does support the self-report of non-compliance with the standards or law and violations were issued. There were 54 children (infants-5yrs.) directly supervised by 8 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group activities, reading books, playing outside, and eating lunch. There was an adequate supply of materials for the children. The center was clean and organized. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.
