Inspector Notes

An unannounced monitoring inspection was conducted today from 10:00am-12:15pm. There were 49 children (infants-5yrs.) directly supervised by 10 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing with small manipulative toys, learning letters, playing outside, and eating snack. Diapering and hand-washing procedures were also observed. There was an adequate supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice.

Standard 8VAC20-780-270-A
Based on observation, areas of the center were not maintained in a safe and operable condition.

Evidence:
1.In the Discovery Preschool classroom, peeling paint, approximately 6 inches in diameter, was observed under the children's cubbies.
2.In the School-age classroom, the girls bathroom is missing a 12x12 piece of floor tile.
Plan of Correction: On 10/18/22 Center Director spoke with the family and requested an Allergy Plan to be completed by the child's pediatrician. The completed form should be ready by 11/02/22.

Based on observation, cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.

Evidence:
In the Discovery Preschool classroom, a bottle of disinfectant cleaner, a spray bottle of multi-surface cleaner and a spray bottle labeled restroom cleaner were observed stored on a high shelf inside the classroom's bathroom.
Plan of Correction: The Lead Teacher in the Discovery Preschool classroom immediately placed all spray bottles away in a locked closet.
Center Director had one on one conversations with all staff members to remind them that chemicals must be stored in a locked compartment away from the reach of children at all times when not in use.
Managers will closely monitor all classrooms to ensure cleaning materials are kept in a locked place.
Standard 8VAC20-780-60-A-8
Based on record review and interview, a written care plan including instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction was not obtained for a child with a diagnosed food allergy.

Evidence:
Child #4's record does not contain a written allergy care plan for their diagnosed food allergy.
Plan of Correction: Work orders had been opened prior to licensing visit. On 10/18/2022, the Center Director followed up with KinderCare technician to ensure requests are completed as soon as possible. Both work orders will be completed by 11/02/22.
