During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 10/29/2024, as a part of the licensure period. The on-site inspection began at 10:00am and ended at 12:45pm. The inspector reviewed compliance in the areas listed above. There were 97 children present and 19 staff. The inspector reviewed 8 children?s records and 5 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-3
The center must request a search of the central registry prior to the employee's first day of employment.
Staff #1, who had been employed for over 90 days and staff #2 who has been employed for over 60 days, did not have documentation of central registry search being requested or documentation of follow up from the center regarding results not being returned.
Plan of Correction: Center to process/contact OBI to obtain clearances and document follow up
Standard 8VAC20-780-130-E
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.

Child #5 did not have documentation of additional immunizations every six months between 18 and 24 months old.
Plan of Correction: Center to request updated immunizations. Center will update immunization records to track dates prior to child turning 2 years old.
Standard 8VAC20-780-260-B
After the first license, annual approval from the health department shall be provided, or approvals of a plan of correction, for meeting requirements for water supply and sewage system.
The center did not have documentation of annual water supply and sewage system inspection.
Plan of Correction: Center in contact with health department to request inspection.
Standard 8VAC20-780-510-P
When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center.

3:5 medications inspected had medication authorizations that expired over 14 days ago, medications were on site and parents had not been notified.

Nonprescription drugs and over-the-counter skin products shall not be kept or used beyond the expiration date of the product.

5:24 diaper creams inspected were kept beyond the expiration date.
Plan of Correction: Center staff to return to parents at pick up.
