Inspector Notes

An unannounced monitoring inspection was conducted on 5/18/22 from 11:25am to 12:45pm. There were 76 (Infants to Kindergarteners) directly supervised by 12 staff. The physical plant, 2 staff records, 5 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group and individual activities, preparing for and eating lunch and taking naps. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Based on review, the facility did not ensure that documentation of children's injuries were included the required information.
Evidence: Reviewed 10 children's injury reports and found that 9 of them did not have documentation of the time parents were notified of the injury.
