Inspector Notes

An unannounced monitoring inspection was conducted 10/27/2022 from 3:20pm. There were (infants to 5 years) directly supervised by 00 staff. The physical plant, 7 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, playing outside. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Evidence: reviewed 7 children's records and found that Child 1, 2, and 5 did not have documentation of the parents' annual review of the child's emergency contact information, completed within the past 12 months.
