Inspector Notes
An unannounced renewal inspection was conducted 3/13/2023 from 2:40pm to 4:10pm. There were 97 (infants to pre-k) directly supervised by 17 staff. The physical plant, 4 staff records, 5 children?s records, 0 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, playing outside, waking up from nap, and getting picked up to go home. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Violations
4Evidence:
1) observed peeling paint in the SACC classroom, 2s room, and the infants classroom
2) observed sharp broken edges around the windowsill in the SACC classroom
3) observed a broken tray on a riding tricycle on the preschool playground
Evidence: the facility did not have documentation available for review on the annual update of the injury prevention plan.
Evidence: observed tears in the diaper changing pad in the infants and 2s classroom, exposing the absorbent material beneath.
Evidence: observed that the most recent posted inspection of the facility was dated 5/18/22 and the most recent recorded inspection was dated 10/27/22.
