An unannounced monitoring inspection was conducted from 9:30 am - 12:30 pm with the center director. There were 96 children, ranging from eight months to 11 years of age, present with 16 staff supervising. Infant care was observed and older children were observed playing outside on the playground, participating in graduation practice, playing in structured learning centers, and engaged in circle time activities. Eight child records and five staff records were reviewed, along with the written daily attendance, written allergy list, emergency drill log, and eight medications and authorization forms. Based on review of five staff records, it was determined that Out of State background checks were not conducted as required for staff who indicated they have lived outside of Virginia in the past five years. Evidence: the record for staff #4, (Date of hire: 9/15/2022), who indicated that they have lived out of the state of Virginia in the past five years, had no documentation of an out of child abuse and neglect search request.

(Repeat Violation) Based on a review of eight child records, the facility did not obtain documentation that each child has received immunizations prior to the child's first attendance. Evidence: the record for child #1 (date of first attendance: 1/21/2024) and child #2 (date of first attendance: 1/22/2023) did not have documentation of immunizations.

Based on review of five staff records, it was determined that not all staff had documentation of a negative tuberculosis (TB) test/screening completed at least every two years from the date of the initial test or screening. Evidence: the record for staff #5 had a negative TB test/screening dated: 4/6/2022.
