During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 10/21/2024 and completed on 10/21/2024. The on-site inspection began at 11:00am and ended at 2:00pm. The inspector reviewed compliance in the areas listed above. There were 88 children present and 13 staff. The inspector reviewed 8 children?s records and 5 staff records on site on 10/21/2024. This inspection included document review, a tour of the facility, observations and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/4/2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-140-A
(Repeat Violation) Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #1, who had been enrolled for 10 months.

Standard 8VAC20-780-260-A
An annual fire inspection report is required from the fire official having jurisdiction.
The annual fire inspection report is overdue by 5 months.
Plan of Correction: Fire department has been contacted six times in the five months to schedule visit. No date will be given.
Standard 8VAC20-780-350-B-2
The required ratio for children ages 16 to 24 month is one staff per five children. Three staff were observed supervising 18 children in the 2?s class. The two youngest children in the group are 23 months of age.
Plan of Correction: Child has an assessment and transition letter on file to meet ratio requirements.
Standard 8VAC20-780-380-A-2
Centers operating five or more hours per day shall have a designated rest period for at least one hour, where children use cots or mats. The preschool aged children in the transitional kindergarten (TK) classroom do not have a one hour rest period using cots or rest mats.

A nonabsorbent surface for diapering/changing shall be used. For children younger than 3 years, this surface shall be a changing table. The children in the 2?s class have their diapers/pullups changed standing up in the bathroom.

Prescription medications are to be maintained in the original, labeled container.
Medication for Child #9 was not in the original labeled prescription box.
