Inspector Notes

Today an unannounced renewal inspection took place between the hours of 10:50am and 2:00pm. There were 6 classrooms observed with a total of 72 children with 11 staff within the supervision guidelines. The children were observed playing freely, transition from the playground to indoors, washing hands before eating lunch and transitioning to naptime. A complete inspection of the physical plant, children and staff records, medications and emergency procedures were observed during this inspection. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified on the Violation Notice.

Based on record review, the center did not obtain additional immunizations once every six months for children under the age of two years.
Evidence: Child #2 (19 months) most recent documented immunizations are dated from when the child was 6 months old.

Based on observation and interview, individuals who do not meet staff qualifications were not within sight and sound supervision of a staff member when in the presence of children.
Evidence: Two children (age 2 years and age 5 years) were observed receiving therapy sessions in a vacant classroom. Each child was observed separately with the therapists without the supervision of a staff member.

Repeat Violation
Based on observation, areas and equipment inside the center was not maintained in a clean, safe and operating condition.
Evidence: All of the corners of the red couch in the 3's classroom are torn and the internal foam is exposed.

Based on observation, restrooms did not contain all required supplies.
Evidence:
1. One bathroom in the TK-B classroom did not contain paper towels.
2. One bathroom in the Pre-K classroom did not contain toiler paper.

Based on record review, the center did not maintain a current written list of all childrens allergies sensitivities and dietary restrictions.
Evidence: Child #6 has a diagnosed food allergy and is not included on the allergy list posted in the classrooms.

Based on observation and interview, the center did not ensure that vehicles used for the transportation of children meet the safety standards set by the Department of Motor Vehicles.
Evidence: Two our of three of the centers vehicles used for transporting children to and from the public schools inspection expired in August 2023. Center management confirmed that the buses were being used and an inspection had not be completed.
