During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 4/23/2025, as a part of the licensure period. The on-site inspection began at 12pm and ended at 3:50pm. The inspector reviewed compliance in the areas listed above. There were 114 children present and 22 staff. The inspector reviewed 10 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-4
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. Staff #1, employed for 6 weeks, did not have documentation of requesting a criminal history record check from one state.
Plan of Correction: The center will provide documentation that the center has requested the information within 10 days.
Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. The immunizations for Child #3 were obtained a month after the child began attending the center.
Plan of Correction: The center will ensure immunizations are received prior to first day.
Standard 8VAC20-780-420-E-3
Staff shall request at least annually parent confirmation that the required information in the child's record is up to date. Up-to-date information is required to be requested from the parent for each child's record at least annually. The information in the record of child #8 had not been updated in over two years.
Plan of Correction: The center will request that the parents review the child's information.
Standard 8VAC20-780-440-E
Centers are required to ensure there are at least 12 inches of space between occupied cots. Multiple occupied cots in two classrooms were observed placed closer than 12 inches apart.
Plan of Correction: The cots were moved during the inspection.
Standard 8VAC20-780-60-A
Centers are required to maintain a record for each child enrolled that contains all required information. The record for Child #6 did not contain the name, phone number and address for 1 of 2 designated people to call in an emergency if a parent cannot be reached.
Plan of Correction: The center will request the information from the parents.
