Inspector Notes

A monitoring inspection was conducted on August 30, 2022. There were 59 children present, with 18 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The children were observed in centers, playing and eating lunch which was beef or chicken with broccoli teriyaki, brown rice and mixed fruit. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-780-370-5
Based on observation, infants, who cannot turn themselves over and are awake, are not being placed on their stomachs a total of 30 minutes each day.

Evidence
Review of electronic documentation, 8/25, 8/26, 8/29 and 8/30, for infant #1 (date of birth 6/6/22) daily activities did not list any tummy time.
Plan of Correction: We will begin recording tummy time for all infants that need it.
Standard 8VAC20-780-500-B
Based on discussion with staff, children under the age of 3, who are in the process of toilet training are not being changed on a nonabsorbent surface for diapering or changing.
Evidence:
Staff #1 confirmed, that children under the age of 2 who are in the process of toilet training are being changed standing up in the bathroom. A nonabsorbent surface is not being used.
Plan of Correction: We will begin using a nonabsorbent pad immediately to change children under the age of 3.
Standard 8VAC20-780-550-P
Based on review of written record of children's serious and minor injuries, 3 out of 5 records did not included all the required information.
Evidence
Three of the reviewed incident/accident reports did not contain all the required information. Missing included the steps to be taken to prevent reoccurrence and how the parent was notified.
Plan of Correction: We will review each incident/accident reports to ensure they are complete before they are signed for approval.
