Standard 8VAC20-780-130-B
Based on review of children's records, not all records contained the required updated immunizations.

Evidence:
1) Child #2 DOB: 8/23/2022 did not have an updated immunization record available for review. Immunization record in file was dated 10/31/2022.
Plan of Correction: We will obtain a copy of the child's updated immunization record by the end of February.
Standard 8VAC20-780-280-B
Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1) Hazardous substances were accessible to children in an unlocked storage closet.
2) Hand sanitizer, disinfectant wipes and disinfectant cleaner (keep out of reach of children) were observed in an unlocked cabinet in the pre-k classroom and swifter wet jet cleaner was observed in the pre-k bathroom.
3) Swifter wet jet cleaner was observed in the Infant 2 classroom bathroom.
Plan of Correction: We will ensure all cabinets are locked. Corrected during inspection.
Standard 8VAC20-780-520-C
Based on review of children's record 3 diaper creams were observed without having parental authorization.

Evidence:
3 diaper creams were observed in the toddler classroom without parental permission for child #1, child #2 and child #3.
Plan of Correction: We will have the parents complete diaper cream forms by the end of the week.
