Inspector Notes

An unannounced, on-site monitoring inspection was conducted 8/1/2024 from 11:10am - 3:20pm. The inspector reviewed compliance in the areas listed above. There were 82 children present and 19 staff. The inspector reviewed 8 children?s records and 5 staff records on-site. This inspection included document review (i.e. Injury logs, policies and procedures, emergency drill logs, training logs, medication administration logs, authorization and permission forms), interviews, observations, and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the
violation notice issued to the program.

Standard 8VAC20-780-270-A
Based on observation and interview, areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition.

Evidence:
1) In Infant 1, there is chipped paint and wood on the windowsill that is indicative of biting.
2) In Infant 2, 2 out of 3 vinyl floor mats are worn and peeling. The mirror with the padded green border has an approximate two inch tear which is exposing the internal foam padding.
3) In Brown Bears, there is chipped paint/drywall in the bathroom around the trashcan.
4) In Busy Bees, there is chipped and peeling paint by the cots.
5) In Brown Bears and Busy Bees, the partition in the bathroom is rusted along the bottom edge.
6) In the outdoor area going to the playground, there is chipped paint and splintering wood around the windows and door frames. There are multiple protruding nails along the wooden fence and gate that could entangle clothing or snag skin.

Standard 8VAC20-780-280-H
Based on observation, harmful agents were stored in areas that are accessible to children.

Evidence:
In the School Age classroom, cans of spray paint, in which the manufacturer guidelines indicate "keep out of reach of children", were stored on a child accessible shelf in the children's bathroom.

Standard 8VAC20-780-440-I
Based on observation, cribs were placed where objects outside the crib such as cords from blinds or curtains were within reach of infants.

Evidence:
In the Infant 1 classroom, a child was observed sleeping in a crib in front of a window in which the cord from the blinds was within reach.

Standard 8VAC20-780-510-I
Based on observation and interview, the center may administer prescription medication that would normally be administered by a parent or guardian to a child provided the drugs were dispensed from a pharmacy and maintained in the original, labeled container.

Evidence:
Child #1 has an EpiPen. The prescription label was removed from the box and was not provided with the medication.
