During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced monitoring inspection was conducted on 08/26/2025 from 12:10pm to 1:42pm. There were 55 children and 19 staff present. The inspector reviewed 5 child records and 3 staff records on-site.The inspection included document review, interviews, observations, and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business 9/3/2025. A POC submitted after this date will not appear on the public website.

Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following: work phone number and place of employment for each parent who has custody; name, address, and phone number of two designated people to call in an emergency if the parent cannot be reached.

The record for Child #1, enrolled for two weeks, and Child #2, enrolled for three weeks, did not contain a work phone number for each parent and an address for one emergency contact.
Plan of Correction: Parents have been notified to provide missing information.
