During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated on 9/19/2025 and completed on 10/1/2025 in response to a self-report received by the licensing office on 9/12/2025 relating to forbidden actions. The inspection included document and video review and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to behavior guidance and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/8/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-400-B
Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior.

Staff #1 was observed using physical punishment towards Child #1, age 21 months, after the child had spit.
Plan of Correction: The incident was self-reported by the school director and the involved staff member was removed from the care of children. The staff members employment was subsequently terminated. Remaining staff have been reminded of positive behavioral guidance policies and the requirement to maintain compliance with 8VAC20-780(6)-400-B.
Standard 8VAC20-780-410-1
The following actions or threats thereof are forbidden: physical punishment, striking or roughly handling a child.

Video footage from 9/10/2025 showed Staff #1 physically punishing a child. Child #1, age 21 months, had partially removed their shirt and approached Staff #1 who was sitting in a child size chair. Staff #1 grabbed the shirt and was attempting to put it back on when the child spit. Staff #1 was seen raising their open palm next to the childs face and pausing. Staff #1 then popped the child on the cheek twice and underneath the chin twice. Staff #1 tapped the child on the lips with their finger once and then went to the sink to wash their own hands.
Plan of Correction: The involved employee was removed from the care of children and their employment was terminated. Remaining staff have been reminded of forbidden actions when interacting with children.
