During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced monitoring inspection was initiated on 1/23/2025 and completed on 1/28/2025 in response to a self-report received by the licensing office on 1/21/2025 relating to forbidden actions and behavioral guidance. The inspector reviewed 1 child?s record and 1 staff record on-site on 1/23/2025. This inspection included document review and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 2/19/2025. A POC submitted after this date will not appear on the public website.
Violations
2On 1/16/2025, Staff #1 did not provide appropriate and constructive behavioral guidance to Child #1 when correcting a behavior. Staff #1 stated to Child #1 "I don't care about your stupid puzzle. We need to go have a chat." Staff #1 took Child #1 to an office alone and closed the door. Screaming, crying, and slapping sounds were heard through the closed door with Child #1 making statements including "ow", "stop hitting me", and "don't hurt me".
The actions outlined took place from 1/17/25 - 2/7/25.
On 1/16/2025, Staff #1 struck Child #1 on the face. There was a visible mark on Child #1's face as a result of the incident.
The offending staff member is no longer an employee of FCCC.
