Inspector Notes

An unannounced monitoring inspection was conducted today with the site manager, who was present with two additional staff and nineteen schoolagers. The inspection was conducted from 4:05 pm to 5 pm. The children were preparing to go outside at the beginning of the inspection. The children spent most of the inspection outside making chalk designs on the sidewalks, playing tabletop activities and walking around the play area. One child had two emergency medications on site, two of the three staff had Medication Administration Training. Eight children and three staff files were reviewed. The areas of non compliance are outlined on the violation notice.

Standard 8VAC20-780-420-A
Based on a review of recordkeeping and interviews, it was determined that parents were not provided in writing, before the children's first day of attendance, policies and procedures outlined under this standard.
Evidence:
Parents for child A through H were not provided the center's policies and procedures in writing.
Plan of Correction: We will provide a copy of the policies and procedures in writing to all the parents.
Standard 8VAC20-780-90-A
Based on a review of eight children's files and interviews with administrative staff, it was determined that the center had not obtained a written agreement between the parent and center for each child by the first day of attendance to include authorization to seek emergency medical care, pick up if the child becomes ill and reporting contagious reportable illnesses to the center.
Evidence:
There were no written agreements for child A through H. The children's first day of attendance was on 8/10/22.
Plan of Correction: We will have the parents to sign the agreement.
