Inspector Notes

The Licensing Inspector initiated and completed an unannounced monitoring inspection on 4/26/2023. The inspector was on site from 8:45am until 11:30am.

There were 50 children under the care of 8 staff. The ages of the children ranged from six months to five years old. The children were observed during indoor free play and outdoor play. Five children?s record and four staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on record review and interview, the center failed to ensure that no person shall be hired for compensated employment prior to the facility obtaining an employment eligibility letter upon the competition of a fingerprint background check as required by the Code of Virginia.

Evidence: Staff #4, hire date of 11/14/2022 had fingerprint eligibility letter was dated 11/16/2022
Plan of Correction: Human Resource Director will ensure that fingerprint is back before staff is hired.

Based on record review, the center failed to ensure each staff member has a documented TB test or screening every two years.

Evidence: Staff #3 has a hire date of12/19/2022. The TB screening on file was dated for 12/21/2022, after employment.
Plan of Correction: Human Resource Director will ensure that TB is completed by hire date.
