Inspector Notes
An unannounced monitoring inspection was initiated and completed on 08/24/2023. The inspector was on site from 12:35pm until 3:00pm.
There were 45 children, ages 5 months to 4 years old under the supervision of 9 staff. Children were observed during naptime and snack. Five children's records, one medical record and four staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations
5Evidence:
Upon review of annual reports the last inspection on record was dated for 8/9/2022.
Evidence:
1.In the afterschool game room, the silver wire protecter is hanging from the wall. This is a entrapment hazard to children.
2. On the entrance of the building, there is rotted wood and exposed nails on the beam leading into the school. This is a snagging hazard to children.
Evidence:
The sandbox on the playground did not have a cover.
Evidence:
When asked the diapering process for children under two, staff stated there is no changing table located in the building for the children to be changed. They must take the children to the other building to be changed. Two-year-olds were observed in the lower building.
Evidence:
There was no documentation of emergency drills on record for May, June or July of 2023.
