During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 2/26/2025, as a part of licensure period. The on-site inspection began at 9:45am and ended at 1:15pm. The inspector reviewed compliance in the areas listed above. There were 62 children present and 11 staff. The inspector reviewed 5 children?s records, one medical record and 5 staff records on-site.

This inspection included document review, and a tour of the facility.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/12/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-130-E
The center is required to obtain documentation of additional immunizations once every six months for children under the age of two years.

Child 3 with an enrollment date of 10/24/22, had no updated immunizations in the records.
Child 4 with an enrollment date of 7/1/24, had no updated immunization in the records.
Plan of Correction: The Director will get updated immunizations from child #3 and #4 by 3-12.
Standard 8VAC20-780-140-A
Repeat violation
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There was no physical examination in the record of child #5, who had been enrolled for two months.
Plan of Correction: The Director will ensure that all children will have physical within the 30 days after first day of school. The child #5 will have form in by 3-12 or sooner.
Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

The wooden post on the outside of the main entrance door is rotting away. Nails are exposed and the opening in a snagging hazard for children.
In the preschool area there was a window that in cracked from the outside. Glass was observed in the window sill of this window on the inside.
Plan of Correction: Will have contractor to look at this to get it fixed.
Standard 8VAC20-780-40-M
The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential.

There was no allergy list available in any of the classrooms.
Plan of Correction: This has been corrected.
Standard 8VAC20-780-60-A-8
A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Child 6, has a diagnosed food allergy, the care plan in their file had expired.
Plan of Correction: This has been corrected.
