Inspector Notes

An unannounced monitoring inspection was conducted on 10/24/2023 from 3:50 pm to 5:20 pm. At the time of entrance, 21 children, ages 5 to 10 years old, were in care with 2 staff members present. The sample size consisted of 5 children?s records and 2 staff records. Children were observed during snack time and involved in free choice table top activities. Parent pick-up procedures and restroom procedures, hand washing and table sanitizing were observed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

Based on a review of records and interview, the licensee did not ensure that a complete record, containing all required information, is maintianed for each enrolled child.
Evidence:
Five children's records were reviewed.
The record for child #1 listed the child's mother and father as the emergency contacts in child #1's record.
The emergency contacts are required to be individuals, other than the child's parents, who could be called in an emergency if the child's parents cannot be reached in an emergency situation.
Plan of Correction: The program site supervisor will ask the parents of child #1 to designate 2 individuals who may be contacted in an emergency if the parents can not be reached during an emergency and document the individuals' contact information in the child's enrollment paperwork / record.
