During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced on-site monitoring inspection was initiated on 9/17/2024 and completed on 9/20/2024. The onsite inspection began at 3:40 pm and ended at 5:10 pm. The inspector reviewed compliance in the areas listed above. There were 26 children present and 3 staff. The inspector reviewed 5 children?s records on-site and 2 staff records electronically on 9/20/2024.
The inspection included document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations are documented on the violation notice issued to the program.

Standard 22.1-289.011-F
Repeat Violation. The license and any other documents required shall be posted in a conspicuous place on licensed premises. The findings from the most recent inspection, dated 5/23/2024 were not posted in a conspicuous place on the premises.
Plan of Correction: The site director stated that she would request a copy of the most recent inspection from the Senior Childcare Director to be posted and will follow up with her to request a copy of each inspection to post if she does not receive the inspection results following future inspections.
Standard 8VAC20-780-240-B
Repeat Violation. Staff are to complete orientation training appropriate to the age of the children in care and including all required facility specific topics prior to working alone with children and no later than 7 days of the date of assuming job responsibilities. Staff #1?s record did not contain documentation of this training which is overdue by 8 months.
Plan of Correction: The Senior Childcare Director stated that the Orientation training will be completed and documented in staff #1's record within 2 weeks. Orientation training will be conducted within 7 days of hire for new staff and documented clearly in their record.
Standard 8VAC20-780-290-A-3
Repeat Violation. In areas used by preschool-age or younger children, electrical outlets shall have protective covers. Children present for care in the cafeteria included a four-year-old preschool child. None of the outlets around the perimeter of the room had protective covers.
Plan of Correction: The site director will request that a large quantity of electrical outlet covers be purchased and kept on hand so that she may replace them as needed since the area where they operate is a shared space and the outlet covers are often removed while the program is not operating.
