Inspector Notes
An unannounced complaint inspection was conducted regarding concerns of physical plant. This visit was conducted on 09/28/2022. The inspection included interviews with the staff and an inspection of the playground. Based on the preponderance of evidence gathered, violations were cited. Based on observation and staff interview, areas and equipment of the center, outside, were not maintained in a clean, safe and operable condition.
Evidence:
1. There was a rusty pipe located towards the center of the sandbox in the ?Cherry Pink Space.?
2. Staff #1 stated that there is a plan to cut down the pipe, add a rubber top and replenish the sandbox with more sand.
