Inspector Notes
An unannounced Monitoring Inspection took place on 03/14/2023 between the hours of approximately 2:25 p.m. and 4:20 p.m. There were 5 classrooms observed with a total of 59 children with 7 staff within the supervision guidelines. The children were observed napping, waking from nap and preparing to go outside. Positive interaction between staff and children were observed. A complete inspection of the physical plant, children and staff records, fire drill log, and playground was conducted during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
5Evidence:
1. Staff #1 (employment date: 01/09/2023) resided in Maryland (MD) in the preceding five years. There was no documentation of a MD central registry finding on file.
2. Staff #1 resided in Washington D.C.in the preceding five years. Staff #4 stated that a search of the child abuse and neglect registry from Washington D.C. had not been requested.
3. Staff #1 did not have the results of a criminal history record information check from Washington D.C. on file. Staff #4 stated that a criminal history record information check from Washington D.C. had not been requested.
Evidence:
1. Staff #1 (employment date: 01/09/2023) did not have documentation of a Virginia central registry finding at the time of inspection.
2. Staff #3 (employment date: 12/16/2022) did not have documentation of a Virginia central registry finding at the time of inspection.
3. Staff #4 stated that the request for search of central registry was submitted during the first week of February.
Evidence: Staff #3 did not have documentation of a negative TB screening on file.
Evidence: There was a wire sticking out from the wire fence that surrounds the Nicholson playground.
