Inspector Notes

An unannounced inspection was initiated on July 31, 2024 and completed on August 6, 2024 in response to a complaint received by the licensing office on July 29, 2024 relating to allegations of supervision and physical plant. The inspector reviewed 1 staff record on-site. This inspection included document review, tour of the facility, interviews and observations. The preponderance of evidence gathered during the investigation supports the allegation(s) regarding supervision; therefore, the complaint is determined to be valid. Information gathered during the inspection determined noncompliance with applicable standards or law and the violations are documented on the violation notice issued to the program. Though not a part of the original complaint, violations that were not reported were found and are documented
on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and returned it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials, shall be kept in a locked place. There were cleaning supplies in an unlocked cabinet and in an unlocked drawer in the green dragonflies classroom. There was a spray bottle containing a bleach/water solution on a cubby in the blue dragonflies classroom.
Plan of Correction: Provide training for all staff on the importance of securing hazardous materials and adhering to safety protocols. Emphasize the potential risks associated with accessible cleaning supplies.

Implement a key management system to ensure that teacher cabinet keys remain in the classroom and are not removed. This includes designated area for key storage when not in use.

Children under 10 years of age always shall be within actual sight and sound supervision of staff. Child #1 (age: 2 years) was out of actual sight and sound supervision of staff on the playground for approximately 2 to 4 minutes.
Plan of Correction: Conduct a mandatory training session for all staff members focusing on supervision policies and the importance of maintaining sight and sound supervision of all children.

Review specific scenarios that could lead to lapses in supervision and strategies to prevent hem.

The licensee shall ensure that the center?s activities, services and facilities are maintained in compliance with the center?s own policies and procedures. The center has a policy that states ?students should be counted at the beginning and end of all transitions.? Staff #1 did not count students at the beginning and end of a transition from the playground to the classroom.
Plan of Correction: Conduct a training session for all staff to reinforce the importance of adhering to center policies, specifically focusing on transition procedures and counting protocols.
Review the center?s policies and procedures with all staff to ensure clarity and understanding. Distribute written copies of the transition policy for staff reference.

If sunscreen is used, it shall be inaccessible to children under five years of age. There were multiple sunscreens in an unlocked cabinet, accessible to children (ages: 4 to 5 years).
