Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interviews and a virtual tour of the program.

An annual subsidy health and safety inspection (SHSI) was initiated on 09/29/2021 and concluded on 09/29/2021. There were 4 children present with 2 staff supervising. A code compliance inspection was also conducted on this date.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the program.

Standard 22VAC40-665-530-2-b
Based on record review, the center did not ensure one of two staff records contained documentation of a child abuse and neglect search request from any state in which the individual has resided in the past five years.

Evidence:
1. The record of staff #1 (start date: 8/15/2021) did not have documentation of a child abuse and neglect search request from any state in which the individual has resided in the past five years.
Plan of Correction: I will get the form in for the out of state background checks.
Standard 22VAC40-665-530-2-c
Based on record review, the center did not ensure one of two staff records contained documentation of criminal history record results and a sex offender registry check results from any state in which the individual has resided in the past five years.
Evidence:
1. The record of staff #1 (start date: 8/15/2021) did not have documentation of criminal history record results and sex offender registry check results from any state in which the individual has resided in the past five years.
Plan of Correction: I will get the form in for the out of state background check.
Standard 22VAC40-665-580-K
Based on record review and an interview, the center did not ensure to have at least one person on staff that has been trained to conduct daily simple health screenings of children.
Evidence:
During the inspection of two staff records and an interview with the center representative, it was discovered there were no staff that received training in daily health screenings within the past three years. Religious exempt child day center requirements stipulate that a staff should be able to conduct a daily simple health screening and exclusion of sick children by a person trained to perform such screenings. Daily health screenings should be completed every three years.
Plan of Correction: I will take the training by the 8th.
