This inspection was conducted by licensing staff.

A complaint was received by the department regarding allegations in the areas of Staffing and Supervision and Special Care Provisions and Emergencies. An investigation was conducted by licensing staff and included interviews, review of documentation and facility inspection.

The complainant alleged the presence of a weapon in the facility. The evidence gathered during the investigation did not support the allegation.

Also alleged was non-compliance with standards regarding injury reporting and records. The evidence gathered during the investigations indicated non-compliance with the Standards and violations were cited.

Standard 8VAC20-780-550-O
Based upon review of documentation and interviews, the facility has not ensured that a parent is notified by the end of the day of any know injuries to their child.
Evidence:
1. The written injury record log provided by the facility indicated an injury to child 1 occurred on 11/09/2021 at 2:30 PM. The same injury record log indicated that the parent was notified on11/09/2021 at 8:45 AM.
2. Interviews revealed that the parent of child 1 spoke to the facility staff in the morning on 11/10/2021 to inquire as to the child's injury.
3. Staff interview revealed that staff documented child 1's injury in a log on 11/9/2021, however staff did not notify the parent at pick-up the same day as the injury.
Plan of Correction: The facility responded with the following:
The injury record log will be carefully completed the same day as a child's injury. Parents will be notified of the child's minor injury the same day the injury occurred and this will be immediately documented on the record log.

Based upon review of documentation, the facility has not ensured that information on the written record of children's injuries is maintained with entries made the same day of occurrence of the injury.
Evidence:
The written record log of the injury of child 1 dated 11/9/2021 at 2:30 PM indicated that the parent was notified of the child's injury on 11/9/21 at 8:45 AM. Information entered does not verify same day recording on the record.
