An unannounced, on-site monitoring inspection was initiated on December 2, 2024 and completed on December 2, 2024. The on-site inspection began at 12:35pm and ended at 2:40pm. The inspector reviewed compliance in the areas listed above. There were 45 children present and 9 staff. The inspector reviewed 7 children?s records and 6 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/16/2024. A POC submitted after this date will not appear on the public website.

Areas and equipment of the center, inside and outside, shall be maintained in an operable safe and condition. Peeling paint was observed in various classrooms within reach of children in the center.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. A bottle of hand sanitizer with the label ?keep out of reach of children? was unlocked on a shelf accessible to children.

In areas used by children of preschool age or younger, electrical outlets shall have protective covers that are of a size that cannot be swallowed by children. An outlet in the preschool room did not have a protective cover.
