Inspector Notes

An unannounced monitoring was conducted on April 18, 2023. There were 98 children present upon arrival with 13 staff members. Additional administrative staff were present. Children were observed napping, eating snack and transition from nap. There was 5 staff records and 5 children records
reviewed . Licensing Inspector reviewed indoor areas, supervision, activities, equipment, emergency evacuation and sheltering-in-place drill documentation, emergency procedures and required posting.

Information gathered during the inspection determined noncompliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on records review, the licensee did not ensure each child shall have a physical examination by or under the direction of a physician: Before the child's attendance; or within 30 days after the first day of attendance.
