Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
An unannounced focused monitoring inspection was initiated on 4/8/2021 and concluded on 4/8/2021 as a result of the issuance of a Provisional License. The director was contacted by telephone to initiate the inspection. There were 16 children present and 7 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on record review, the center did not obtain immunization records for a child prior to the child's first day of attendance.

Evidence:
Child#2's file does not contain immunization records. The child's first day of attendance was documented as 3/28/2021.
Plan of Correction: Spoke with family about issue. Family turned in documents following day. Now in compliance.

Based on record review, children's records did not all contain required information.

Evidence:
Child#1's record did not contain the father's contact information. Child#2's record did not contain the street number for 1 emergency contact.
Plan of Correction: Spoke with child's family about issue at hand. Family filled out forms with information. Now in compliance.
