An unannounced monitoring inspection was initiated and completed on 2/3/2025 in response to a self-report received by the licensing office on 1/27/2025 relating to behavior management. The inspector reviewed 1 child?s record and 1 staff records on-site. This inspection included ocument review of Injury/incident report logs, policies and procedures, tour of the facility, interviews, and observations.
