Inspector Notes

An unannounced monitoring inspection was initiated and completed on 8/19/2025 in response to a self-report received by the licensing office on 8/8/2025 relating to forbidden actions. The inspector reviewed 3 children?s records and 1 staff record on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to background checks and are documented on the violation notice issued to the program.

The center is required to have employees undergo a background check every five years.

The fingerprint background checks for staff #1 was completed 5 months, after due date.
Plan of Correction: Employee fingerprint background check completed in 6/2025 when it was noticed during staff file audit. Staff files will routinely be audited and corrections documented.
