Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
1. In the 2?s B classroom, a smashed pumpkin left over from the day before, was found on the bathroom floor, used by children.
2. The second toilet in the 2?s B classroom was being used a storage shelf and not able to be used.

Plan of Correction: The used supply (pumpkin) has been thrown away. Toilet is ready for use.

Cleaning supplies (sanitizing solution) was in the bathroom, used by children in the 2?s B classroom, not locked and accessible to children.
