Inspector Notes
A complaint inspection was initiated on May 15, 2023 in response to a complaint that was received on May 3, 2023 in reference to behavior management and positive child guidance. Video footage was viewed on May 17, 2023. Documentation was gathered and interviews were conducted with local agencies. The preponderance of evidence gathered during the investigation supports violation of standards regarding the complaint.
Violations were cited.
Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 06/15/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).
Violations
4Evidence: The record for staff #1, date of employment 12/6/21, did not contain documentation demonstrating completion of recognizing child abuse and neglect and the legal requirements for reporting suspected child abuse as required to be included in orientation training by 8VAC20-780-240 subsection C.
1) Video footage reviewed revealed that on 05/02/23 staff #1 grabbed toddler child #1 by the arm and forcefully walked the child to the table and sat the child down.
2) Video footage revealed that on 05/03/23 staff #1 grabbed toddler child #1 by the arm and forcefully walked the child to the table, sat the child down, and forced the chair to slide into the table hitting the child?s stomach. Approximately one minute later staff #1 is observed throwing a bib on top of child #1?s head which covers the child?s face and obstructs their view, and leaving it there while staff #1 helps another child with their bib.
Evidence:
1) Video footage reviewed revealed that on 05/02/23 staff #1 grabbed toddler child #1 by the arm and forcefully walked the child to the table and sat the child down.
2) Video footage revealed that on 05/03/23 staff #1 grabbed toddler child #1 by the arm and forcefully walked the child to the table, sat the child down, and forced the chair to slide into the table hitting the child?s stomach.
