Inspector Notes
A complaint inspection was initiated on August 4, 2023 in response to a complaint that was received on July 18, 2023 in reference to a child injury. Documentation and video footage was reviewed and interviews were conducted. The preponderance of evidence gathered during the investigation supports violation of standards regarding the complaint.
Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 08/24/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).
Violations
1Evidence:
Interviews with staff #1, staff #2, staff #3, and staff #4 and review of video footage revealed that on July 17th, child #1 (age 5 years 5 months) was observed improperly riding a tricycle which caused child #1 to flip backwards and hit child #1's head on the chain link fence. Child #1 required a visit to the emergency room and received medical intervention.
Observation and measurement of the tricycle revealed that it is approximately 10 inches in height. Documentation review of the manufacturers label revealed that the tricycle is designed for children ages 3 years to 4 years.
